Instructions for Completing Health Forms 2006-2007

Our school is subject to inspection by the Health Department and if parents and/or students are participating in our program without these forms, CPI would be closed immediately. If health forms are not RECIEVED by the Health and Safety Director before the due dates, your child's enrollment may be forfeited and a child from the waitlist will be selected to fill that spot. This policy will be strictly enforced as it has become a severe compliance issue. It is advised that forms be carefully read and filled out as any incomplete form must be returned and this creates a delay in processing and could jeopardize your child’s spot.

We regret that the BLUE Medical form is not available through the CPI website. It is a county form available through your doctor or pediatrician's office. You may also request a copy of this form from Parent Education. This form is due: Tuesday, August 1, 2006.
